The WP-Connect team provides care navigation and resource connection to service members and their family members at Wright-Patterson Air Force Base. 

• The team is staffed with military veterans who are experienced in helping to identify needs, developing a plan, providing care navigation services, helping to address any barriers to moving forward. 
• They can assist with navigation and connection to on-base and community-based resources. 
• For service members and family members that are already engaged in some services, care navigation can  complement existing services such as a mental and physical healthcare by helping to address other needs. 
• Services are confidential.
